The term is "pre-existing condition", and depending on the policy,
pre-existing conditions are usually covered after a certain amount of time,
depending on the policy and the state laws of the state the policy is issued
in.
Also, if he hasn't been treated for the problem previously, then if he were
to go in and say that "Last week I was wrassling cattle (or whatever) and my
shoulder started to hurt..." there would be no pre-existing condition on
record, and it would be filed as a new injury, triggered by whatever the
incident was.
