Fred,
I have a waiver and hold harmless agreement that I did last year for my
wife's hot glass classes. I had my old law partner check and revise it so I
think that it is pretty comprehensive. I'd be happy to give you a copy (MS
Word document). Of course laws vary from state to state, I no longer have
professional liability insurance so I am not even suggesting that this is a
good legal document. It might be a start though if you want to do your own.
Best bet is to check it with your local attorney.
You could also get a lot of good forms and advice by doing a Google search
for "hold harmless agreements".
