Gabriel-
Put Seattle Pottery Supply (Hanford Street) on your list as they have most
everything you need for gas forges.  If you want ITC-100 or satanite to coat
your kao-wool they have that in addition to all types of cast refractory.
They also have budget thermocouples on up to spendy digital ones if you care
to see where your forge is running.
Some of the staff actually have a clue about what forge-guys need, like
high-alumina castable for flux resistance. 
You may want to go with Kaowool for your first forge but the castable will
last much longer. It's worth the extra effort to make a form out of an old
carpet or sono-tube and pour one yourself.
My latest forge uses a rigid kao-wool liner with about an inch or so of
castable on top of it. Works well and stands up to the abuse of having
things scraping the sides in there.

For decent hoses, fittings and regulators- Evergreen RV supply up on the
160th block of aurora avenue has plenty of stuff in one location.
