  This was done by an animal registry organization that I belonged to 20 years
ago for voting for officers. It's probably the standard procedure:
  You use two envelopes, one smaller than the other. Just the ballot goes into
the smaller envelope and is sealed. Both the slip with the name and address
and the smaller, sealed ballot envelope are enclosed in the large envelope
which was addressed to an independent auditing firm. 
  When opened, the name and address slip is used to check who had voted and
that no one had voted more than once. The sealed envelopes containing the
ballots are thrown in a pile and opened later, opened by someone else, or
whatever, to tally the vote. Voila, secret ballot by mail, with name and
address slips available separately.
