Here is how I was shown, no math involved.
Roll your handrail the hard way but not tight to the lay out curve, it will
tighten up when you twist it.
Using forks, hold in the middle and at one end, twist the bottom in(down)
and the top out(up). This make the twist.
Elevate the section with one end on the floor and the other at the hieght
you finish at on the stair.(The slope angle.) Sight one end to the other, if
the ends are parallel you have the right twist.
 Working on curved stairs takes you to the next level in house work. A lot
of shops back away from this work. We start at $350/ foot for curved work.
 You can model this effect with a piece of pipe and a lead flat bar. The
tighter the diameter and the higher the elevation, the more twist that is
needed.
 On very tight rails , we build a full sized jig. Have a drum rolled to the
diameter you need. Mark the slope with a chalk line.Weld angle iron clips to
the line. Heat and bend your material, clamping to the angle iron clips as
you go with vise grips. I hope this helps.

The rail for a curved stairway has a twist in it as wellas a curve.
Do any of you have any good ideas for doing this twist?
I've done several of these, the curve is easy, make a pattern from the
stairs or lay out the curve on the shop floor and bend the iron to it.
The twisting is where I always have trouble.  I go to war w/ big wrenches
etc. and fight for hours to get it right.
