This is the results of trying this.

There are some problems with the entries left in the form. Check the
following information in the areas highlighted:

The item number is a number
The dash number is a number
The code is 3 letters
We have no record of that item (N/A

I think something is wrong with the "CP8".  Looks like it should be 3
letters.
