I was not able to tell from your post whether or not you had 'back-seated'
the valve on the propane tank.
Along with oxygen tank valves, inert gas tank valves and some acetylene
valves, back seating valves are designed to be used in the full open {and I
mean opened to the point of definite resistance} or the full off position.
The act of opening the valve only part way invites 'problems', i.e. gas
leaks, around the stem. most common problems with old valves are buggered
stems and non sealing valves in the off position. Some large tank valves
also have a 90% valve or port that can be opened while filling so that the
operator can tell physically when the tank is 90% filled by virtue of the
discharge of liquid propane from the vent when the tank is filled correctly,
it is basically a dip tube. this is not affected by the tank valve, but if
present make sure it does not leak.
It is prudent to test all gas piping before it is placed into service, and
all testing should be done using leak testing solutions. Your kids' bubble
solution is ideal, or just mix ivory dish soap  1 or 2 ounces
karo syrup  1 ounce
about 10 ounces of water
place in a spray bottle and saturate the joint that are in the gas stream,
both yours and any 'factory' joint that you can reach.
Fire is hot, play with caution.

Yesterday I picked up a 100# tank I had recertified. this evening I
connected my regulator to it aand opend the valve on the tank. I felt some
"air" blowing on my hand so I took the regulator off and but on an extention
line and opened the valve felt it again so tank will go back to the place
that certified it for a new valve.
Valve leaking around stem is what I think and I was willing to pay for the
new valve when they had it the first time.
the Tank had not been used in years but had a little gas in it so I guess
they thought it OK.
