Doug -

I have access to a very large, and quite old, hardware store, and I 
would begin there. I have, in the past, been able to find sleeves for 
similar purposes. I have drilled and tapped holes in the sleeves, and 
used very short set screws to attach them to the inner shaft.

If I cannot find something in hardware locally my go-to online is 
McMaster-Carr.
