Just speculation on my part, but I've read on the Antique Radio Forum that after Fluke stopped production of the militarized 27/FM, the military continued to buy some of the commercial version Fluke 27's. Possibly the PSM-45 is the real Fluke 27/FM (with the true-RMS chip) and the PSM-45A is just the commercial Fluke 27 (no true RMS).
