I probably would not use a MAV-11 for this purpose nowadays.  It's an older
part and may not offer as much gain as you need.

It might be worth looking at a newer MMIC such as the GALI series on the
second page at http://www.mini-circuits.com/dg03-166.pdf .

Regardless of what MMIC you choose, an RF choke with good broadband Q and a
high self-resonant frequency is needed to maintain flatness and stability.
You can use a resistor by itself, but it's better to have one or more chokes
in series with it.
