Try a search on voice tagger or pair identifier.
 HP used to make one-maybe Agilent still do- the 4960 I think-  and 3M
probably still have one available. No doubt lots of others as well with
varying maximum identifiable pair numbers etc.
 A smaller one is here- maybe will do what you want-

Does anyone know of a hardware device that will allow you to attach a
tone or digital ID sender at one end of a cable and that ID can be
picked up at the far end of the cable. Not a tone gen and amplifier,
this device would connect to say 20 wire ends at once and send a unique
code/Id down each wire to be read individually at the near end? We are
talking telecoms cable of 30 pairs of wire @ 30,000ft.
