Geeze
I read that. Several times... before your most current instruction too... BUT  
it still does not explain if you want the request submitted to the Bricks and mortar address by mail? OR send the information by E-Mail. Also Do you need a vanilla envelope?  Im just trying to get it right. And clear directions avoid's mistakes and confusion. 
Or at least they did when I was still a 911 and emergency Services center supervisor.
None of that is explained as far as I can determine.IE.
