Ah, Paul.  I see the problem.  Your doctor thought you wanted to visit a
*Mental* Hospital.  The paperwork you received is designed to provide an
initial assessment of your perceptive state.  

You should have received the other form, the one with stuff like "Number of
teeth in sockets, Number of teeth in pockets" and all that sort of good
stuff.
