Two good ones that I've used before trips to "the other side of the pileups"
are:
G4ZFE Pile Up! program  http://www.g4zfe.com/pileup95.html
Great program, but:
Requirements
Pile Up! requires:
�       Windows 95. The program will not work on Windows 3.1 or Windows NT;
�       Sound Blaster compatible card. Note that only ISA sound cards are 
fully supported by this version.

PED CW Pileup trainer  http://plaza16.mbn.or.jp/~masiii/
It emulates CT logger.

With PileUp! you can get credit for partial calls.  You get points for 
every character in the call.  With PED you need the full call and an 
exchange - it is like CQ WW CW.  After some practice with 2 or 3 calls at 
once you can get up over 100/hour, but it's tough to sustain higher 
rates.  There is also an SSB version of the Pileup trainer.

I've never become really good at either so as to keep my soc intact!
