[SJRA-Members] Looking for .01UF/10nF/10000pF 4kv Ceramic Disc Capacitor for Amp Supply LA-1000 KW Amplifier

Looking for capacitor in subject line.  Although it looks fine in the 
picture the lead is broken off inside the capacitor so it cannot be 
fixed.  I need to replace it and they do not seem common these days.
