Not that I'm aware of it, its in the article.  You might be able to find it 
on the members only portion of the ARRL Web?  Otherwise I could scan it 
later if need be, I was thinking of making a blown up copy of it for the 
wall so I can double check it against my records here.  One of these days I 
need to get the cards sorted and get a VUCC cert on the wall.
