It is quite simple, go to _http://www.6mt.com/sixapp.htm_ 
(http://www.6mt.com/sixapp.htm)  and  fill out an application and it would help if you paid using 
the paypal link  after you submit the info (meaning you will have to back up 
to the app page and  then click the paypal link).
 
If you don't use paypal I will send you a email with additional info.
