Andy,

"Against Medical Advice."  The paramedics would like the patient to go
to the hospital and try to explain the reasons why a visit would be
preferable, however the patient is under no obligation to take the ride.
 The waiver is a legal release holding the rescue agency harmless in
case the patient suffers any consequences from not taking that ride.
