[Scan-DC] Similar incidents - RE: Here We Go Again

There was a crash on 6 August where the craft was observed descending flat
from 24,000 feet (fuel tanks empty, the plane on autopilot?). Perhaps there
is a pressurization problem, or a consistent human error.

There was another incident where the pilot turned off the fuel instead of
