The ones I have heard involved planes that entered restricted airspace or 
didn't have the proper squawk transmitting.  In one case it came to a 
dispute between controllers at BWI and National as to whether  the 
restricted airspace had been violated.  In some cases the newspaper 
explanation was more detailed than what I heard (included followup info 
from interviewing the pilot).  In most cases the published info was more brief.
