I hereby certify under penalty of perjury that:
1. I possess a valid Federal Communications Commission Amateur
Radio Operators License, a copy of which is attached.

2. The vehicle described above contains or carries a transmitting
and receiving unit of a type capable to that of my class of
Amateur Radio License and the transceiver is capable of
operating on at least 5 bands from 160 through 10 meters.
3. An antenna, a power supply and wiring are a permanent part of
the vehicle.
4. The license I hold is a GENERAL class, or higher.
5. I will immediately, upon deletion of equipment or transfer of
equipment to another vehicle, notify the Division of Motor
Vehicles and surrender my amateur radio license plates or
transfer them to another vehicle.
6. I understand that the equipment may be removed from my
vehicle only for service or dry storage but the wiring and power
supply are a permanent part of the vehicle.

design, the $30.00 personalized plate fee is charged.

Complete the Certificate of Eligibility in Full. If you certify to the
first statements, all normal fees are due. If you certify to all
statements, you will not be charged for the registration fees. All
other fees still apply.

STATIONS DUE TO THE PAPERWORK REQUIREMENTS.
