[Scan-DC] Airplane Question

I have a few quick questions about when people post military aircraft
information:

1.  Are there any frequencies where pilots dont talk in code, but in regular
English?

2.  How far away can you hear an airborne object?  E.g.:  Can you hear
planes over New York, West Virginia?

3.  Whats the farthest plane (commercial or military) anyone has heard on
their scanner while in the Wash DC area?

4.  Are there any websites where you can find out what the codes and numbers
mean that I hear on the radio (military and commercial please).
