[SADXA] DX Ladder clarification

The consensus of opinion is that the Ladder should reflect confirmed, 
current entities only: no Czechoslovakia, East Germany, Canal Zone and 
the like.  That means if you're using the LotW Account Status screen, 
the needed totals come from the "Total (Current)" column.  Using this 
column makes it easy to report.  No recounting is necessary, since 
pending LotW confirmations are already included.

The DXCC Challenge totals will be published for all those who send them 
over.  The spreadsheet itself will be sorted by the "Mixed" scores, from 
high to low.
