[SADXA] DX Ladder data needed

Dick, there were several conversations going on at the same time.  I 
think one side of the room had settled on DXCC Challenge totals, while 
the other was in favor of Mixed totals.  Let's see if we can resolve 
this via this email thread.

What's the consensus of opinion from our members:

1. rank order the list via Mixed totals or Challenge totals?

2. in listing the band, mode, and mixed totals should confirmations for 
deleted entities (e.g., Canal Zone, Czechoslovakia, East Germany, etc.) 
be counted or not?
