You can get silkscreening kits in various sizes from your local       
graphic arts supplier. Speedball makes, or used to make, a nice one.  
Also, you most likely can get a local silk-screener to make you up    
a framed screen; all you need to do then is get the image stencil     
into the screen, which is pretty much the same process as for making  
photo-etched PCBs.                                                    

It's a *lot* of trouble to go to for a one-off, though.
