You can go to the QCWA website and  find the documents to apply.
That site is http://www.qcwa.org/

At the bottom under Membership, you will find applications.  You
can apply either online or download the application and mail it
to headquarters.

If you have other questions post them and someone will answer.

Sir, I qualified to enroll for the QCWA in 2001 but I have been
going through a lot of rehab so didn't have much time to get on
the air. Well, I am through with my rehab for a while so I want
to get back on the road again. Where and how can I sign up for
QCWA?
