Hi Bill - No big problem.  You will be listed in the 1951 call book.  Jim
Walsh will look that up in the QCWA library.  That's all the proof that you
need.  Go to www.qcwa.org.  You will find all the necessary info there.  I'm
sure there will be an application that you can download.  If you don't find
it just let me know and I'll mail (or FAX) a copy to you.
