[PVRCNC] cootie key

Hey Tom,
A cootie key is manual dits and dahs, where you continuously alternate
between the left and right paddles for each element, left is not
necessarily dits, and right is not necessarily dahs. No keyer circuit
required, just like with a bug or straight key.
