Probably no "one" did this. someone has spyware on their machine and it watched as they connected to your site. It recorded the username and password and then sent the information to a central "bad guy" location.  The central bad guy then proceeds to put the htaccess file onto your ftp site (and probably sub folders) in an automated process. I had this happen with my own website a few years ago.  The spyware was on my own machine (I am the only one logging in to make changes to my website). once I cleaned up my PC this stopped happening.  So one of your customers (or you) that accesses your FTP site has a virus/spyware/malware nastiness on their machine.  To reiterate, I had the exact same problem you describe (.htaccess) and I discovered the source of this business was my own pc.  Hope you can resolve it soon.

I just finished rebuilding the PCS FTP site. Someone planted htaccess in the directories and erased contents. If this happens again I will change the passwords. The PCS FTP site is federally monitored. We record the IP addresses of individuals who access the site. An investigation has been triggered.

Please, do not upload anything without my permission. The consequences are actionable.
