I generally make up my own cables with a ratcheting crimp tool, but have 
purchased cables from Pasternak for test equipment. Not cheap, but good 
quality.

Alternatives I would consider are:

I've dealt with both The Wireman and The RF Connection for parts but 
both will assemble custom cables for you. You can spec the cable type, 
length and connector quality and ask them to give you a price.
