Before you dismiss all synthesisers completely have a look at the (now
nearly 5? year old) design of the cdg2000 transceiver that was published
in the RSGB journal Radcom:

 http://www.warc.org.uk/cdg2000/The%20CDG2000%20Transceiver.htm

The synth in this, which is single loop with a lowish frequency DDS as
reference, is capable of state of the art performance, (phase noise
graphs are on the web site) and the design could presumably be adapted
for MW use if it were desired, but the 3 kHz wide roofing filters might
be a little on the narrow side for AM :^).

Mine, which I really must finish one of these days, has a front end
performance equal to or better than the latest commercial offerings.
The overall design could benefit from some DSP processing after the IF,
but that has been promised by the designers, and we live in hope!
