One secret seems to be liberal use of isolation amplifiers.  See figure 1 in
http://www.mini-circuits.com/appnote/mxr1-18.pdf (a good app note in
general).  Cheaper MMIC-based amps probably won't help much, because of
their poor reverse isolation.
