Check with George Kerber at www.glkinst.com for the cables.  There will be
some sticker shock, but George does good work.

You can also order the D-shell connectors and internal coaxial connectors
from Mouser Electronics and make your own, but I've long since lost track of
the part numbers.  (If I remember right, George was the one who gave me that
hint in the first place...)
