[PPRAANet] Anyone fill out a FCC 605 form for a vanity callsign?

I have the paperwork to help my Dad apply for a vanity call but it is in a  
code I don't know. If you have dealt with the FCC 605 could you contact me and  
help me decipher this stuff.
