[PaQSO] What info in realty needed for the summary sheet?

For most (I'd say >99.44%) of contest I've entered in the past few 
years, except for the "honor system ones" I've generally not bothered to 
fill in any of the scoring details if there's summary sheet requested.

However, I notice that Pennsylvania REQUIRES a summary sheet or an entry 
won't be counted.  Do I need to calculate my score?  I've been told that 
claimed score is only "advisory" or a "Brag sheet" item, so I generally 
just take whatever score my logging software calculates, or sometimes 
change the number to zero before submitting it.

Since I'm one of the highest rollers from Rockford, Michigan, I want to 
be sure my entry is valid so I can claim the trophy for being the winner 
from this zip code.  That's why I want to be sure I "get it right."  
(Right?  I did read somewhere in the rules or otherwise that there's a 
special trophy for the highest score in zip code 49341-9502.  Or was I 
dreaming?)
