[PaQSO] Summary Sheet not Fillable

Same problem using Windows 7.  I was going to do the same as Bud however I am able fill in the copy Dave WN4AFP sent over.

I was unable to fill it in online or after downloading it.  I am using Windows 10.

 

I printed it out, filled it in by hand, scanned it, and sent the resulting PDF.

I have tried three different computers all with Adobe Reader 11.0.12 and it does not allow any fields to be filled.  Reader pops up an error that the extended features are no longer available.  The summary sheet in the package is fillable.  I was not sure if the PA QSO folks want the whole 19 page package file sent.
