[PaQSO] Important Summary sheet announcement

The original summary sheet posted on the website must have been corrupted in some manner.

I've fixed the problem,, and you should be able to open, enter data, calculate and sign as needed in Acrobat reader.

The new file is named NewSummary14a.pdf
