Compile the missing information with what was mailed, get it to me via 
email and I will gladly generate an Adobe Acrobat (.pdf) file for the NARC.

Once the .pdf is generated, I will send it back to the NARC, let them double-
check it for integrity, then post a much smaller-sized file to their web page.  
The .pdf file will be able to be downloaded and even printed just like it is 
received in the mail.

This is a standing offer.  If anyone from the NARC is interested please 
contact me via email first.
