[OMIK] OMIK 62th Anniversary Convention Information and Registration Form

Thanks for the information Steve!  I will not be able to do any extensive travelling for quite some time.  However, I lost contact with OMIK, and as consequently I could not pay dues.  Please provide information that will enable me to be reinstated.
