Gee folks ..I did  not want to get a rhubarb started BUT I don't see ANY
requirement to be an ARRL approved checker in this form!
I stated in the orignal e-mail this was for OMISS.  Guess "we" get fixed on
DXCC ARRL and can't remember this is a HOBBY!

OMISS VOLUNTEER QSL CARD VERIFICATION FORM

This form may be used for verification of QSL Cards for all awards and
endorsements offered by the OMISS that require QSL Card Verification. You
may have your ****QSL Cards verified by any two volunteer Amateur Operators
who hold the license class of General or higher****.  This form may then be
submitted in lieu of sending QSL Cards and return postage to the OMISS
Awards Manager.  You must still submit the Award Application and Log Entry
form, plus this form.  Please carefully follow the instructions below to
avoid your application being sent back to you for any necessary corrections.
Note:  The Awards Manager may still call for a spot-check of any cards of
any applicant for any award for any reason the Awards Manager feels
necessary.  Failure to comply with such a courtesy check will result in the
return of your application.

To the Volunteer Card Checkers:  Thank you for helping a fellow ham submit
for an award with the OMISS Net.  You will be saving the station some
postage, in addition to the peace of mind knowing that their valuable QSL
Cards will not be forever lost in the US Postal system.  You should be
presented with a Log Entry Sheet with all of the calls dates, times, and
other related information of the calls qualifying for the award the member
is applying for. This log should match the QSL Cards which the member will
submit to you for verification.  Please carefully  inspect the Log Entry
Sheet against the QSL Cards to be sure of the following:

1.  All cards are present and accurately match the Log Entry Sheet.
2.  All cards are addressed to the station applying for the award.
3.  There are no erasures, line-outs, or write-overs of any of the logged
information on the QSL Cards.
4.  There is no missing information or blanks not filled in on the QSL
Cards.
