[NLRS] Does anybody have a nice PDF of a good log sheet?

For some reason I can't find the PDF of the logging sheet we've used in
past contests.  If somebody has a good one, could you please send me a copy?

The KC9JTL/W9HQ rover-mobile is about 90% ready to go.  We'll put the
finishing touches on things in the morning and will send out an e-mail when
we're wheels up and on the road to the starting grid for the contest.
