[NLRS] Attention uW ops: ARRL Committee Seeks Microwave Band Plan Input

The "Input Form" that they have at this page:
http://www.arrl.org/microwave-band-plan-input-form
would seem to be the easiest to fill and to process. You just fill in
the page and hit submit, then back up and re-fill the appropriate lines
for another band. It would also seem to be much easier to process at
the League's end, rather than spreadsheets that some people will
undoubtedly re-format, forget to attach, or whatever.
