I do not know.
The instructions say "Use a separate page for each band for which you wish to provide input". I suggest that this means they want a separate file for each band ... for example, if you were going to respond for all four bands in this frequency range, I would replicate their spreadsheet four times and use one file for each band, then send them all four files. I would not replicate the form as a new worksheet (tab) within one file as they may not see that ... if you are not looking for worksheet tabs its easy to miss them. Yes, one file with multiple worksheet tabs makes more sense, but its not as easy to see. My thinking is that they view the form as a "page" ... if they had used the work "worksheet" or "tab" rather than "page" then the one file approach would be good, but they didnt.
In summary, I suggest going with a separate Excel file for each band.

Thanks for letting us know about this. I do have a question in case you are able to answer.
The request asks for a separate page for each band. Does this mean to copy the existing page in the spreadsheet and make a new page in the spreadsheet for each band and then paste the format from the first page? And then fill the info in for each band on these additional pages?
Or should we make a separate Excel file for each band?
I myself would prefer the first above.

An ARRL Ad Hoc Committee has been tasked by the Board of Directors with recommending updates to the ARRL band plans for the amateur bands between 902 MHz and 3.5 GHz. If you are now active on any of these bands or are developing plans to do so, the committee would like to hear from you.
At the link provided below, towards the bottom of that page, you will find links to a PDF and to a Excel form. The PDF contains more detailed information along with directions about how to fill out the Excel form. Reading and filling out the form should take about five minutes.

Please respond to this request for input by December 15, 2011.
