[NLRS] NLRS Shirts -- 2nd Try

Ok folks, we've got shirts !  (Well, we can order shirts.)

Thanks to tips from people at Aurora '11, I think we've got
a winner this time.

The shirts are 100% cotton, 7 oz knit sports shirts.   Color
is Dark Green  (it may not be a perfect match to our
existing shirts, but its close.  Besides, the ones we have
are 6 years old... they may have faded a bit.)
All shirts will have the NLRS logo embroidered on the upper
left chest.  As an (extra cost) option, you can get your
Call and Name embroidered on the upper right.  Also
available (extra cost) is a pocket on the left chest.

Yes, the price has gone up from 6 years ago, but not as much
as the first quote (and reported on at Aurora).

Costs:
Shirt w/NLRS logo
XS - S - M - L - XL -
    XXL - XXXL                                  $25

Add for Pocket                                  $ 2
Add for Call and Name
  on right chest                                   $ 5

We do need a minimum order of 25 shirts to make the order
work.

So, what I'd like to do is this:
If you want to order a shirt (or two, or...), E-mail me with
your order ASAP.  Follow that up with a check, cash, etc.,
to me (Donn Baker, WA2VOI, 3128 Silver Lake Rd NE,
Minneapolis, MN 55418-2426).  I'd REALLY like to get the
order in by the 15th of this month (June).  I need to have
AT LEAST a committment by the end of Friday, June 10th, with
cash to follow immeadiatly.

I don't know about size/fits other than what the catalog
says;

XS         32-34        14-1/2 -15
S            35-37        15 - 15-1/2
M           38-40         15-1/2 - 16
L            41-43         16 - 16-1/2
XL         44-46         16-1/2 - 17
XXL      47-49          17 - 17-1/2
XXXL   50-53          17-1/2 - 18

I've checked, and the "Slashed-0," (Ø) will be used for
Calls.

Your order should state:
-- The NUMBER of shirts ordered  @ $25 each
-- The SIZE of (each) shirt ordered
--  Whether or not you want a pocket @ $2 for each shirt
--  Whether or not you want Call / Name @ 5 for each shirt
--  IF YOU ORDER CALL / NAME
        you MUST specify your Call and Name
        (Call will be top line, Name will be 2nd line,)

EXAMPLES:
1 Shirt, XL, w/ Pocket, no call/Name    $25 + $2 = $27 total

1 Shirt, L, no pocket, Call = NØABC, Name = Alan  $25 + $5 =
$30

2 Shirts, XL, pocket, Call = NØXYZ, Name = John  $25 + $25 +
$2 + $2 + $10 + $10 = $74
    (Both shirts will be IDENTICAL)
    IF you want differences, order the shirts individually.
