[NLRS] NLRS POLO SHIRTS ORDER

Several weeks ago, I polled NLRS to see if there was enough interest in getting
club shirts to warrent an order.  There was !  Although there were some problems
getting the details settled, we're now ready to take actual orders !!

The shirts are the same design as before: Forest Green with the NLRS logo
embroidered on the left breast.  Sizes are Small, Medium, Large, X-Large,
XX-Large, and XXX-Large.  Cost changes slightly with size:  Small, Medium, Large
and X-Large are $17;  XX-Large is $19; and XXX-Large is $20.  They are available
with or without a pocket on the left breast.  There is NO charge for the pocket.

PLUS there's an option of having one or two lines embroidered on the right
breast for Call and/or Name.  Call on the top line and Name (First name or
nick-name) on the second line.  There IS a $3 charge for the Call/Name option:
either one line or two lines, its $3.

We are ready to collect FIRM ORDERS and MONEY.  (The order needs to be
pre-paid.)  I'll collect orders and money, consolidate everything and send a
single check to the company.  The order will go in on Nov 1st.  WIth a little
luck, we'll have the shirts in hand for distribution at the December Breakfast,
which is December 11th.

I will need your order and payment NO LATER THAN October 31st for it to be
included.  Thats TWO WEEKS from now, so don't delay !

Below is an Order Form for the NLRS Shirts.  Print or Copy the Order Form and
send or give to me, WITH FULL PAYMENT (you can make checks out to me --(Donn
Baker - WA2VOI) on or before October 31st 2010.  I'll send a single check to the
company.

Please fill out an ORDER FORM for EACH shirt ordered: (EX: two shirts,
two order forms, even if the shirts are identical)

Select the size (note that some sizes cost slightly more than others).

Select whether or not you want a pocket (no charge for pockets).

Select if you want your Call and/or Name embroidered on the right breast.
Carefully PRINT EXACTLY what you want for Call / Name on the order form.
    You may have Call only, or Name only, or Both.
    There is an ADDITIONAL CHARGE of $3 for Call and/or Name.

SEND (mail, or give to me) the completed order form (or forms) along with full
payment on or before October 31st 2010.  (And no, I won't take "Tricks or
Treats" !)

PLEASE drop me an e-mail (wa2voi -at- arrl -dot- net) letting me know you're
sending an order so I can be looking for it, especially if there' s any question
that it might not make it on time.  (Plan on 4-5 days for mail to arrive here.)

For those of you we can't hand deliver shirts to, there will be a slight
additional charge (actual costs) to get your shirt(s) mailed to you.  The
"shipping" item on the order form is for getting the shirt(s) from the
manufacturer to us.
