[NLRS] States Above program

NLRSers,

Jon is actually referencing an "electronic" entry FORM for States Above that 
I "authored" using Excel 2003, so feel free to contact me as an alternate 
source. It IS an Excel Workbook where the contact info is entered (separate 
worksheet for each band) and it automatically fills in the Summary Sheet 
which you can then save as you have updated it and send along to Peter...so 
don't be expecting a "web-based" submission (I *think* it will work at least 
back to Excel 2000, though I have NOT checked that so any feedback will be 
appreciated).
