[NLRS] Radio ??

Can anyone identify this radio??

I am looking for a model # or some kind of identifier, I got the
receiver at a ham fest and resurrected it seems to receive very well it
covers 100-160Mhz I don't have any idea who made the radio but it is
very similar to some Collins construction or maybe RCA its all tubes I
don't recall the tube complement off hand. just trying to find some
information on it. I can receive CW SSB and FM signals just fine so not
even sure what "mode" it was for?

Also I am looking for one of the phone jack covers like what's on this
radio, the flip down covers. They are for a different project so I don't
care what they look like just so they snap shut.
