[NLRS] VHF Survey

I will have copies at the NLRS Breakfast this Saturday (12 Oct) at the
Roseville location. If necessary, I'll supply envelopes and stamps!

You won't be allowed to eat untill you've filled it out, and licked the
envelope !!  :-)
