[NLRS] CSVHF States Above & WAS rules

Hi there Donn:

Good catch on the radius vs. Diameter.  Yes, all the states above rules are
on the CSVHFS web.  I've given the webmaster the updated info but it has not
made it to the pdf or html versions of the rules yet.  The example states
total will show 107 on the latest version once it is there.

clarifications:  CY0 and CY9 count for Nova Scotia, and using VUCC
"location" definition.

You may be surprised how few rules are specifically "States Above".  That's
because ARRL WAS seems to cover most situations.  But the issue I've
addressed is one that I did not even anticipate but was brought to my
attention by those participating in the program.
