Most of your questions are answered in a very fine history of ham radio
operators by Kristen Haring, Ham Radio's Technical Culture (MIT Press,
2007), which is available, astonishingly enough, from Target:

You may notice that the reviews at Amazon vary from QSL's positive to
particularly Patricia Allison's, which rips Professor Haring a new
orifice for the slant of her sources.  But that shouldn't stop you
necessarily from borrowing the book via interlibrary loan.
